Our Vision:

To ensure a Healthy and Aware community, through health promotion, and creating a link between Primary Health Care and the community.

Our Mission

Establising well trained, committed Home Based Carers, Coordinators and sound Board to achieve our vision

We accomplish our vision through daily door-to-door visits, to educate our community in communicable and non-communicable diseases i.e. HIV/AIDs, TB, hypertension, diabetes, etc.

Our staff members / Community Health Workers (CHW) undergo compulsory in-service training to be able to educate clients. These CHW’s are allocated to wards within the community resulting in clients knowing their caregivers and building strong relationships with them.

Our system of screening and referral to Primary Health Care (PHC) ensures that signs and symptoms are caught early and treatment is started at the earliest time. Clients referred back from Primary Health Care facilities, receive treatment adherence support as to reduce number of defaulters.

We are also part of a network of  Non-Profit organizations called the Langeberg Multisectoral Task team, that enables support, training and sharing of resources.

As part of the network we join with different initiatives outside of our core function to benefit our communities. As we believe that to heal our society and our communities, we need to have an holistic approach.
